Job Title: Case Manager
Location: Mountain View, CA
Pay Rate: $3500.00
Duration: 13 Weeks
Shift: Day shift 8 am-4:30 pm Mon-Fri
Care coordination is a collaborative process of assessment, planning, facilitation and advocacy for options and services to meet the individual's health needs through communication and available resources to promote cost-effective outcomes, using evidence-based guidelines as available in daily practice. The Care Coordinator focuses on effective, safe, timely and complete transitions of care. In collaboration with the multi-disciplinary health care team, this position is responsible for the timely coordination of health care services to meet the patient's individual needs and promote quality and cost effective outcomes. Shifts are 8-hours,
1. ASN or diploma in Nursing required
2. Bachelor's degree in nursing, or healthcare related field preferred
3. 3 years of recent experience in the acute care setting.
4. Minimum 1 year experience in case management/case coordination role OR 1 year of *** clinical nursing (RN) current experience
5. Experience in managed care/continuing care preferred.
6. Advanced communication skills in advocacy, negotiation, confrontation and conflict resolution.
7. Computer literate, with experience in word processing, data analysis and presentation development.
License/Certification/Registration Requirements
1. Current California RN license
